The following forms must be completed for each vaccination you wish to receive. If you are bringing eligible dependents with you to be vaccinated, you will need to complete an insurance form for each dependent, and for each vaccination they will be receiving.

FAQ's
- Will any other vaccinations be offered at the Vaccination Clinic?
No. At this time, the only vaccinations being offered are the flu and Tdap vaccinations.
- Is the Vaccination Clinic also open to students?
No. Due to the limited supply of Tdap vaccinations available, we have limited this Vaccination Clinic to eligible employees and their dependents.
- What do I need to bring with me in order to attend?
In order to attend, you must bring with you all completed insurance forms for each vaccination you wish to receive, as well as your valid insurance card. If you are bringing eligible dependents with you to be vaccinated, you will need to complete an insurance form for each dependent, and for each vaccination they will be receiving.
- What if I am not covered under Group Health or Uniform Medical Plan?
Get A Flu Shot.com has provided us with a list of insurance groups regularly accepted for each of the vaccinations. These lists are available for your viewing below. Please note, there is a shorter list of accepted insurance groups for the Tdap vaccination.
If you are not covered by an accepted insurance, the cost for a flu shot is $30 and the cost for a Tdap vaccination is $65. Get A Flu Shot.com accepts cash or check, no credit or debit cards.
- Is there an age limit for dependents?
Yes. Get A Flu Shot.com will vaccinate children as young as 11 years of age with parental consent. They can also vaccinate children as young as 9 years of age with a written pediatrician's order.
- Do I need to schedule an appointment?
No, you will not need to schedule an appointment in order to attend. Vaccinations will be offered on a first-come, first-served basis from 11:00am to 3:00pm.
