Community Fitness Center users need the following to obtain an ID card:
- ID number
- Picture ID (i.e. driver's license)
- Current quarter enrollment in Fitness Center
- Tuition and fees must be paid in full
Enrollment and payment of tuition and fees will be verified each quarter to receive a new quarterly sticker. Stickers may be obtained from the ID counter in Records or from Hawk Central while paying tuition.
Only one ID card will be issued per user. A fee of $3.25 will be charged for replacement of the card. The fee must be paid at Hawk Central prior to receiving a new ID card.
Appropriate picture ID:
- Driver's License
- United States Passport
- Armed Forces ID
- Employee photo ID
